If you, as the owner of the dog has a specific [School ID] or identification number to track each dog in your program(s), you can add this number here.

 

If the Breeder has a specific ID for each dog born into their program, this can be added here. If this field has an ID entered here when you receive a puppy, please leave it in place and enter your school ID in the School ID field above.
